Weiss Functional Impairment Rating Scale

What Is the WFIRS-S?
The Weiss Functional Impairment Rating Scale – Self-Report (WFIRS-S) is a 50-item questionnaire used to evaluate day-to-day challenges in six key areas of life: work, school, relationships, and self-care. It uses a Likert scale and is psychometrically validated.
This tool is designed to measure functional impairment, which is one of the criteria required to diagnose ADHD in both children and adults. While many ADHD assessments focus on traits or behaviors, the WFIRS-S looks specifically at how those traits may impact functioning in everyday settings.
Why We Use It Thoughtfully
The WFIRS-S is based on a deficit model, which we typically avoid. It uses the language of “impairment,” which doesn’t reflect our neurodiversity-affirming approach. We prefer to use terms like “challenges,” “needs,” and “support areas,” and we’re mindful of how language shapes perception.
That said, the WFIRS-S is currently one of the only tools that focuses specifically on functional domains affected by ADHD. And because functioning and traits don’t always align perfectly, this tool can be helpful in understanding how ADHD shows up in real life and how much support may be needed.
We don’t view it as a stand-alone measure or a label of “how impaired” someone is. Instead, it’s just one piece of the larger picture, used to clarify patterns and advocate for meaningful support.
